Intensity-Modulated Radiation Therapy (IMRT) is a highly precise cancer treatment method that utilizes beamlets to deliver targeted radiation doses, aiming to maximize tumor eradication while minimizing damage to surrounding healthy tissues. This study presents an optimization framework for IMRT treatment planning, incorporating a mixed-integer programming model to address dose distribution challenges. We propose three distinct treatment strategies, each obtained by solving a separate Multi-objective mixed-integer program under a different objective and/or set of enforced constraints. The three plans are intended to illustrate alternative trade-offs between tumor coverage and healthy-tissue sparing, rather than to constitute points on a formally derived Pareto front. The formulation employs two alternative single-objective functions: one maximizing the total CTV dose and the other minimizing the total non-CTV dose. The results demonstrate the behavior of the model under these alternative formulations, providing illustrative trade-offs for decision-making; however, no formal multi-objective scalarization or Pareto-based procedure is presented.
